Documents to complete prior to your first appointment:

• Intake form & Privacy and informed consent form - Please complete these forms and return via email or bring to your first appointment. Completing the form at your first appointment is possible, however may cut into your appointment time.

Documents for your information:

• Privacy Policy - Stipulates how your information is used and stored, aligned with current legislation.

• Confidentiality Charter - As our counsellors are registered with the peak body for counselling in Australia, they are expected to adhere to strict ethical guidelines. The charter details what you can expect from a ACA registered counsellor according to these guidelines.
